健康保険証が廃止されるため、マイナンバーカードを早めに作ってください
In accordance with the new national policy, health insurance card and My Number card will be combined into one.
New and reissued health insurance cards which have been used up until now will be expired on December 2nd (Monday).
Please make your My Number card as soon as possible.
■ My Number card issueing procedure
There are several ways to make My Number card, but the following method is the most convenient one.
① Go to window No. 133 on the first floor of the high-rise building of the city hall to apply for your My Number card.
② Your My Number card will be directly delivered to your house by personal mail at a later date.
※It will take about one month and a half from the time you apply for your My Number card to arrive.
■ Required documents
・Notification card or personal number notification
・Identification document (driver’s license, residence card, etc.)
・Basic Resident Registration Card (only if you have one)
・Photo (Could be taken at City Hall. Free of charge.)
