初回産科受診料の一部を助成します
The Suita City will subsidize a portion of the expenses required for a first consultation at an obstetric medial institution, etc for expectant mothers of a low-income household to get a diagnosis of the pregnancy.
￭Eligible people
Suita city residents who are exempt from local taxes or who live under public assistance program.
￭Subsidy amount
First obstetric consultation to receive a pregnancy diagnosis (maximum ¥10,000 per visit)
￭Application method
Choose one of the following ways to apply.
(1) Reimbursement of the first obstetric consultation fee (the consultation can be done at obstetric medical institutions in the city or outside of the city)
You will be reimbursed for the first obstetric consultation fee up to 10,000 yen by paying the expenses yourself first and then applying for the reimbursement later.
(2) Medical examination slip (Available only at cooperating medical institutions in the city)
For those who have difficulty getting a medical examination due to lack of money or other reasons, we will issue a check-up slip that can be used up to 10,000 yen in advance. First, please consult with the Maternal and Child Health Division.
￭Documents to submit
(1) Reimbursement fee for first-time obstetric consultation
① Maternal and child handbook (unnecessary if you don’t have it)
② Receipt (original) and detailed statement
③ A copy of a document (e.g: bank booklet, cash card, etc.)that can confirm the account (financial institution name, branch code, account number, account holder (furigana)) in the name of the expectant mother.
④ Inkan (signature stamp)
⑤ 【※ Only for those who meet the requirements. Applicant’s household tax status certificate.
(e.g. city resident tax statement – Shiminzei Kazei Shoumeisho)
(2) Issuance of the medical examination slip
